Curry Family Medical is accepting new patients.
●Pick up a new patient packet at the clinic.
●Download a new patient packet.
●Call 541.332.3861 to request one be mailed or emailed to you.
●After you have completed the forms in the Packets, you may:
     1) fax them to 541.412.2083
     2) mail them to Curry Health Network, ATTN: Patient Intake Coordinator, 500 5th St., Brookings, OR 97415
     3) scan and email (encrypted and secure) via our online form.

Mon: 8 - 5, closed noon-1 pm
Tue: 8:30 - 5, closed noon-1 pm
Wed: 8:30 - 5, closed noon-1 pm
Thu: 8 - 5, closed noon-1 pm
Fri: 8-4, closed noon-1 pm

Lab hours may vary and patients can call 541-332-3861 to confirm availability. Generally, labs are available Monday through Friday from 8:30 am to 3:30 pm.
