A patient's primary care provider is the leader of your health care, contributing to your medical care and surrounding you with the resources necessary to keep you healthy. In addition to providing essential primary care, an important component of care coordination is referring you to a specialist for services he or she isn't able to provide.Many of Curry Health Network's Specialists require a Referral from a patient's primary care provider, or other physician.
Referring Providers
Providers wishing to refer their patient to a Curry Health Network specialist should download, complete, and fax our Specialty Referral Form, a convenient, fillable PDF that must be faxed to 541.412.2084 along with required supporting documentation.This form may be used to refer your patient to:
